Collect

One 6.0 mL red top no gel, serum (clot activator).

Pediatric Collection

At least 0.5 mL whole blood in a red top no gel, serum (clot activator).

Unacceptable Conditions

Specimen not properly identified; incorrect container; insufficient sample volume.

Storage/Transport Temperature

Internal: Deliver to lab immediately at ambient temperature..
Offsite: Allow to clot 30 minutes, immediately centrifuge , place serum in aliquot container and send at 2-8 degrees C.

Before centrifugation: Ambient, 1 hour.
After centrifugation: Refrigerated, 1 month.
Serum aliquot: Refrigerated, 1 month; Frozen (-20 degrees C), 3 months..

Include dosage information. Draw trough level immediately before the next dose. If toxicity is suspected draw at any time.

Interpretive Data

Effective 05/20/2014:

The expected range is provided only as a guide, and individual patient results should be interpreted in light of other clinical signs and symptoms.  For effective treatment, some patients may require serum levels outside this range.  Peak concentrations above 20 μg/mL are often associated with toxicity.

 

Before 05/20/2014:

Toxic levels of total Phenytoin are greater than 20.0 ug/mL.

Falsely elevated Phenytoin concentrations have been observed in patients with renal deficiency or failure.
