Interpretive Data
Matrix Metalloproteinase-7 (MMP-7) has been proposed as a biomarker in differentiating biliary atresia (BA) from non-BA in pediatric cholestatic patients. An elevated serum concentration of MMP-7 may be consistent with the diagnosis of BA. Elevated MMP-7 values have also been detected in premature babies and young infants with cardiovascular and/or multi-system disease. Normal ranges were verified in full term infants. The proposed cut-offs for MMP-7 as a biomarker for discriminating BA in pediatric cholestatic patients for infants up to 150 days of age are as follows:
0-30 days 28.8 ng/mL 30-150 days 18.5 ng/mL
If the MMP-7 level is at or below the cutoff, the infant should be closely followed until the jaundice clears or the suspicion for BA is eliminated.
Caution should be exercised in the interpretation of any assay for MMP-7, and the results should be considered in conjunction with the patient's clinical condition. The final diagnosis of biliary atresia requires intraoperative cholangiography and/or histological presence of obstruction of the extrahepatic bile duct.
The concentration of MMP-7 in serum or plasma is determined by chemiluminescence immunoassay. The Nephrology Clinical Laboratory at CCHMC has verified the performance of the assay including precision, accuracy, reportable range, linearity, reference range, and sensitivity. It has not been cleared or approved by the U.S. Food and Drug Administration. This laboratory is certified under the Clinical Laboratory Improvements Amendments of 1988 (CLIA) and the College of American Pathologists as qualified to perform high complexity laboratory testing.
