A Rapid On-site Evaluation can be requested by calling the Cytology department located at the performing hospital/location or by calling Customer Service at 317-491-6000.
If specimen is being procured for Precision Genomics, call the cytology department (317-491-6338) for assistance with procurement.  Core biopsy and robust cell block collection is required. 
 

Preferred Draw Volume

Adequate cellular material for cytologic evaluation obtained from an appropriately performed fine-needle aspiration is required. 
This will depend on the specimen site and character of the lesion being aspirated. In general, this requires that there be enough material on the smeared slides for the examiner to at least determine that the aspirating needle sampled a mass/ lesion.  It is best to perform this with the assistance of a cytologist.  Please call 317-491-6338 for assistance.

Cystic fluid: Greater than 1.0 mL

No patient identifiers on slides, syringe (if cystic fluid), or container
Crushed glass slides

Collection Comments

All specimens, including slides, must be labeled with 2 unique patient identifiers.

Direct Smears:
Specimen from the FNA is gently smeared in monolayer onto labeled glass slides. Use single end frosted slides for the preparation of smears. Label each slide with two unique identifiers (patient name and date of birth or MRN) and the source. The method used for making the slide will be determined in part by the nature of the material present. Once the specimen is on the slide, it must be smeared. To accomplish this, oppose a second glass slide onto the first, allowing the aspirated material to provide surface tension between the two slides, then gently and quickly pull the two slides apart in a horizontal or vertical motion to distribute the material in a thin film over both slides. Slides may be allowed to air-dry. Place into a slide holder/container for transport to lab.

Cytology Fixative - CytoRich Red (CRR):
 If direct smears are not prepared, flush all aspirated material into a tube containing cytology fixative such as CytoRich Red. If material remains in the hub of the needle, the hub may be rinsed into a tube containing cytology fixative by pulling the fixative into the syringe then expressing it back into the container. DO NOT LEAVE UNATTACHED NEEDLE IN THE CYTORICH MEDIUM. Label the tube with patient identification.

RPMI:
If history/suspected lymphoma: place 2-3 directed passes into RPMI solution. 

Formalin:
Used to place direct specimen into fixation if history of/suspected breast cancer to comply with ASCO requirements. Place directed specimen into neutral buffered formalin. Label with patient identification, the time the specimen has been placed into formalin (ie 14:00) and place in biohazard bag with the rest of the FNA specimen slides/tubes.  

Cystic fluid:
Fluid sample to be placed directly into Cytology fixative such as Cyto-Rich Red, into a sterile container, or submitted in capped and labeled syringe.

For HPV Genotype testing of head and neck tumors from FNA specimens:
Collect a minimum of three FNA passes in CytoRich Red medium. Indicate on cytology FNA order: FNA supernatant HPV testing.

Microscopic evaluation of cellular morphology from Fine Needle Aspiration (FNA) biopsy is used to help determine if the cause of a mass or lump is a cyst, an infection, or a benign or malignant lesion.  The FNA procedure is often used in conjunction with CT, Ultrasound, and Endoscopy evaluations; however, FNA can also be performed on palpable lesions.

  • A printed Cytology requisition must accompany all slides/tube and containers
