Place appropriate order in Powerchart and complete required fields. This test requires a black and tan Streck glass tube. Supplies are not stocked by Sendouts. Tubes must be obtained through LabCorp. Please call 317-491-6000 to obtain information. Collect one Streck tube. Mix tube 8-10 times to mix anticoagulant with blood. Do Not Spin Tube. Whole blood sample. Remember to properly label tube with patient name, DOB, test and collect date and time. Copies of insurance information must be submitted with he sample. Send all paperowork with sample to IUHPL Sendouts. Ship room temperture. LabCorp courier will pick up at 3:00 PM and 8:00 PM, Monday - Friday.
