After adminstration of any of these compounds, parsites may not be recovered for a several weeks. Collection of stool specimens should be delayed for 5 to 10 days or at least 2 weeks after barium or antibiotics.

4. Stool received in SAF, MIF, Zinc PVA or Copper PVA transport media will be sent to reference lab for testing

5. If a GI FilmArray panel, stool culture, or parasite/viral antigen tests are ordered with the O&P exam, the panel test will be performed and not the individual assays. Contact microbiology laboratory prior to sample collection if individual testing is clinically required.

Stool collected in diaper is acceptable following NCH Helping Hands-Stool collection guidelines. Stool collected in diaper must be placed in appropriate container/media prior to sending to lab.

Stool in contact with urine will not be processed but rejected.

If sample volume <5gm or 5mL, place entire sample in the white capped Total-fix vial or the blue capped PVA vial. Giardia /Cryptosporidium antigen (GCR) recommended as initial screening test. Consider O&P if antigen test is negative and symptoms persist or in patients with foreign travel history. Cryptosporidium is not detected by O&P exam. Request for Cystoisospora (Isospora)and/or Cyclospora spp. order the Send-Out test. For Schistosoma: Urinary schistosomiasis should be considered in individuals who come from areas of endemic infections who have signs of frequent urination, dysuria, terminal haematuria, suprapubic pain, and recurrent urinary tract infections. The terminal hematuria portion (presence of blood only in the last fraction of voided urine)of the urine specimen may contain numerous eggs trapped in the mucus and pus. Peak egg excretion occurs between noon and 3 pm. All 24-h urine specimens that are more than 48 hrs. old will be rejected. Schistosoma examination should not be ordered on a stool sample, only urine. For Larvae of Parasitic Helminths (Worm) indicate on requisition : "Process for Larvae of Parasitic Helminths or Worm". CPA: For Larvae of Parasitic Helminths (Worm) order an OAP and record under SREQ comment "Process for Larvae of Parasitic Helminths (Worm)" or "Process for Worm for Identification".

Inpatient Specimen Preparation

InLab Processing

O&P vials with different collection date/time MUST be ordered as separate tests. For Cystoisospora (Isospora)and/or Cyclospora order the Send-Out test. For Larvae of Parasitic Helminths (Worm)order an OAP and record under SREQ comment "Process for Larvae of Parasitic Helminths (Worm)" or "Process for Worm for Identification". For Schistosoma, Urine collection should be either a midday, or the supernate of a 24 hour urine collection without preservatives. Urine should be examined after concentration by centrifugation. Do not use pneumatic tube system to transport vials.

Additional criteria:

1. Do not accept specimens from inpatients after the fourth hospital day, without prior consultation. Contact microbiology laboratory 722-5370.

2. It is the recommendation that no more than 2 or 3 specimens are collected without prior consultation. Contact microbiology laboratory 722-5370.

3. Certain substances and medications also interfere with detection of intestinal protozoa including barium, mineral oil, bismuth, antibiotics (metronidazole and tetracyclines), antimalarial agents, and nonabsorable antidiarrheal preparations. After adminstration of any of these compounds, parsites may not be recovered for a several weeks. Collection of stool specimens should be delayed for 5 to 10 days or at least 2 weeks after barium or antibiotics.

4. Stool received in SAF, MIF, Zinc PVA or Copper PVA transport media will be sent to reference lab for testing

5. If a GI FilmArray panel, stool culture, or parasite/viral antigen tests are ordered with the O&P exam, the panel test will be performed and not the individual assays. Contact microbiology laboratory prior to sample collection if individual testing is clinically required.

Stool collected in diaper is acceptable following NCH Helping Hands-Stool collection guidelines. Stool collected in diaper must be placed in appropriate container/media prior to sending to lab.

Stool in contact with urine will not be processed but rejected.

If sample volume <5gm or 5mL, place entire sample in the white capped Total-fix vial or the blue capped PVA vial. Giardia /Cryptosporidium antigen (GCR) recommended as initial screening test. Consider O&P if antigen test is negative and symptoms persist or in patients with foreign travel history. Cryptosporidium is not detected by O&P exam. Request for Cystoisospora (Isospora)and/or Cyclospora spp. order the Send-Out test. For Schistosoma: Urinary schistosomiasis should be considered in individuals who come from areas of endemic infections who have signs of frequent urination, dysuria, terminal haematuria, suprapubic pain, and recurrent urinary tract infections. The terminal hematuria portion (presence of blood only in the last fraction of voided urine)of the urine specimen may contain numerous eggs trapped in the mucus and pus. Peak egg excretion occurs between noon and 3 pm. All 24-h urine specimens that are more than 48 hrs. old will be rejected. Schistosoma examination should not be ordered on a stool sample, only urine. For Larvae of Parasitic Helminths (Worm) indicate on requisition : "Process for Larvae of Parasitic Helminths or Worm". CPA: For Larvae of Parasitic Helminths (Worm) order an OAP and record under SREQ comment "Process for Larvae of Parasitic Helminths (Worm)" or "Process for Worm for Identification".
