This test is available for patients (probands) who previously had clinical Genome sequencing or clinical Rapid genome sequencing performed by Nationwide Children's Institute for Genomic Medicine Clinical Laboratory.

Required forms for the PATIENT (Proband/Child)

  • If the patient (child) previously participated in Genome sequencing (GSPAT) then submission of the following documents are required: completed Genetic Test Requisition Form and GS Reanalysis request form. A new sample is not required. A new GS consent form is also not required unless the patient (child) who previously had GS performed is now ≥18 and is able to provide consent.
  • If the patient (child) previously participated in Rapid genome sequencing (RGSPAT) then submission of the following documents are required: completed Genetic Test Requisition Form, GS Reanalysis request form and GS consent form. A new sample is not required.
