Collect

One 6 mL SST (Gold tube) or Red tube.
2.0 mL serum; 0.5 mL minimum.

 

Unacceptable Conditions

Plasma or other body fluids, severely lipemic, contaminated or hemolyzed specimens.

Do not order the RPR test unless one of the following conditions are met. 
If none of the conditions are met, order a Syphilis IgG (SYPIGG).
 
  • Positive treponemal test on the current sample (FDA approved testing such as Syphilis Antibody, TPPA, THA).
OR
  • Previously positive results for at least 2 of the following tests on the same sample:
  1. Treponemal screening test (FDA approved testing such as Syphilis Antibody, TP-PA, THA)
  2. RPR
  3. FTA
OR
  • Patient <6 months of age

Storage/Transport Temperature

Refrigerated. If transport to lab is not the same day as collected, centrifuge the sample and transfer the serum into a clean tube.
 

Stability (from collection to initiation)

Stability: After separation from cells: Ambient: <8 hours; Refrigerated: up to 7 days; For longer storage, freeze at -20 º Celsius.  Avoid repeat freeze/thaw cycles.

Notes

Not diagnostic of syphilis unless paired with a positive treponemal test on the same sample.
Also used to monitor treatment of a previously diagnosed syphilis infection. 
The RPR is a non-treponemal test.

A reactive RPR result will be quantitated (titer).  If the RPR is nonreactive, an FTA will be performed in the absence of documented history.

A non-treponemal test, such as RPR, is recommended for children 6 months of age and younger.
