Specimens that exceed stated stability, unlabeled/mislabeled/mismatched specimens, specimens submitted in leaking containers, specimen without 2 patient identifiers, specimen received without test requisition form, frozen specimen, specimen received in tubes other than sodium heparin (green top), fixed specimen with Formalin or any other fixatives

Special Instructions

Label the specimen tube with patient's name and medical record number and hematology case number (M#). Specimen source is required on test order supplied by hematology lab. Current WBC count for same specimen is required.
