  1. Complete the appropriate Consent for Autopsy:
  • Consent for Autopsy / Autopsy Abstract (Form D556), English
  • Consent for Autopsy / Autopsy Abstract (Form D556S), Spanish
  • Consent for Perinatal Autopsy (Form D3959), English
  • Consent for Perinatal Autopsy (Form D3959S), Spanish
  • Approval for Autopsy on External Decedent (Form D4880), English
  • If you do not have the required form available, visit Pulse - Supply Chain Management for instructions on how to obtain the appropriate form
  1. Attach a green Hold For Autopsy Service (Form D1747) notice to the patient chart
  2. Schedule with Autopsy Service by calling Decedent Affairs at 619-543-2963
  3. Nursing Staff and Security are responsible for escorting the patient to the UC San Diego Health System Morgue Cold Room (Hillcrest Room L-430, Thornton Room LL-311). All bodies delivered to the Morgue/Autopsy Suite must go straight to refrigerated storage and must NOT be left on gurneys in the hallway.
  • The Autopsy Suite does not have an attendant on duty.  All routine matters pertaining to the Suite and/or Cold Room (such as admitting or releasing bodies) will be the responsibility of:
    • Hillcrest: Decedent Affairs and Hospital Security
    • Thornton: Admissions Department and Hospital Security
  1. Security will confirm I.D. and ensure proper storage of the remains for autopsy
  • Hillcrest: Security will transfer the patient to another gurney in the cold room and document placement on the Morgue Log
  • Thornton: Security will place the patient in the appropriate body bay for autopsy

Patient Preparation

Body must be brought to the morgue in a body bag. If a body bag is needed, you may contact Autopsy Service at 619-543-5719.

*NOTE: Special handling instructions are required for COVID decedents, refer to UCSDHP 330.1

Unacceptable Conditions

  1. Bodies not brought to the morgue in a body bag
  2. Bodies received without valid, completed Consent for Autopsy Form
  3. Verbal or telephone consent is unacceptable
  4. Bodies that have been incorrectly admitted
  5. Bodies that are incorrectly identified or unidentified
