Ordering Recommendations

  1. Request through EPIC by selecting PATHOLOGY TISSUE EXAM, selecting the tissue site, and ordering "Placenta"
  • For manual non-EPIC requisitions use Surgical Pathology / Neuropathology Consultation Request (Form D4845).
  1. The order must include the following:
  • Clinical History (MUST include the following information)
    • Gestational Age
    • Singleton vs Other
    • Mode of Delivery
    • Live Birth vs Stillbirth
  • ICD-10 Coding
  • Collection Date and Time
  • Requesting physician’s PID number
  • Indicate if the patient has any infectious disease history or current infections
  1. ALL PATHOLOGY SPECIMENS MUST BE SUBMITTED WITH A PAPER REQUISITION. NO FUTURE OR STANDING ORDERS ARE ACCEPTED.
  2. Specimens will not be processed until adequate identifying and billing information is received. 
  3. ALL Surgical Pathology specimens must be delivered or sent via courier to the SURGICAL PATHOLOGY LAB TISSUE GROSS ROOM in Hillcrest Room 2-304 or Thornton Room 2-016. Normal business hours are Monday - Friday, 8:00 AM - 4:30 PM. ALL SPECIMENS MUST BE LOGGED in the PATHOLOGY DROP-OFF LOG.
  • In-House Deliveries: Placentas are to be taken to the PLACENTA ROOM (Thornton 2-716) and placed on the right hand side in the placenta storage refrigerator until the orders can be placed in EPIC.
    • Once ordered in EPIC, L&D staff will take the placentas to the gross room 2-304
    • If the specimen includes an Infant needed for autopsy, a nurse or other L&D staff member will be escorted by security transport to take the infant and the placenta to the morgue

  1. Collect placenta specimen and immediately place into a Sterile Container
  • Specimen must be submitted fresh. Do NOT place the tissue into formalin.
  1. Ensure that the cap is tightly secured on the Collection Container

Specimen Container Labeling Instructions:
  1. Specimen Containers submitted to Pathology must be labeled with the following patient information:
  • First and Last Name
  • Date of Birth
  • Medical Record Number
  • CSN Number
  • Name of Specimen Site/Source
  1. Label placenta containers with a blue dot sticker for patients with any of the following:
  • All current and recovered COVID Positive patients
  • Patients with any of the following infections:
    • Active tuberculosis
    • Zika exposure during pregnancy
    • HIV
  1. Check all labeling of containers prior to leaving the collection site
  1. Place the following in a biohazard bag for transport to the Pathology Lab:
  • Specimen Container
  • Specimen Requisition
