Collect

Preferred:  Pus from aspiration, bone marrow aspirate, corneal scrapings, wound exudate, drainage fluids, small pieces of tissue or biopsy (maximum 1 inch cubed), venous or arterial catheter tip, bile, gall bladder fluid, prostate tissue or fluid, placenta tissue, or genital swab submitted in a dry, sterile container.

ESwab transport device

For corneal and conjunctival cultures, obtain media from the laboratory for bedside inoculation.

Pediatric Collection

In a sterile container, 0.5-2 mL fluid or 0.1 g viable tissue
or ESwab transport device

Unacceptable Conditions

  • Sample not properly identified
  • Incorrect container
  • Insufficient sample volume
  • Delayed transport to the laboratory
  • Tissue in fixative
  • Urinary catheters
  • Grossly contaminated specimen
  • Swabs are not acceptable for tissue culture.

Storage/Transport Temperature

Ambient
Refrigeration may compromise the recovery of fastidious organisms. Delay in transport may result in overgrowth of contaminating flora.

Stability (from collection to initiation)

For optimal microorganism recovery, deliver specimens to laboratory as soon as possible after collection.
On-site locations: deliver within 2 hours at ambient temperature

Off-site locations: deliver within 12 hours at ambient temperature
Eswab: Deliver to lab as soon as possible; manufacturer claims stability up to 48 hours.
** Specimens not in transport media that are delayed over 24 hours may not be accepted.

Remarks

Aerobic Cultures include abscess, aspirate, catheter tips, deep wounds, pus, surgical wounds and tissues, ear, bone marrow, gastric, skin, sinus, eye, conjunctiva, cornea, drainage, open wounds, lesion, burn, decubitus, incision, ulcer, and genital specimens.

Indicate specific site, including laterality, in the order (e.g. left leg). 
Indicate suspected organism to ensure appropriate plating and incubation. 
For intravascular catheter tips, please indicate the specific catheter type on requisition. Catheter tips should be submitted simultaneously with blood cultures.
If orbital cellulitis is present or suspected, communicate this to the laboratory.
Indicate if Actinomyces is suspected. Actinomyces has been reported in endometritis and pelvic infections associated with IUDs.

rder Anaerobic Culture if anaerobic organisms are suspected. For other requests, order the specific culture type (e.g. Body Fluid Aerobic for peritoneal fluid, Fungus Culture for yeasts and molds, etc.). 
To screen for Neisseria gonorrhoea only in genital cultures, please submit specimen for DNA probe.To screen for Group B Strep, order Group B Strep PCR.

The Aerobic Culture order includes a direct Gram stain on abscesses, aspirates, amniotic fluid, tissue, and Copan eSwabs in transport media. A Gram stain may be performed by request for other miscellaneous sites.

Susceptibility testing is performed on clinically significant aerobic pathogens, if recovered.
Requests for additional work-up may be directed to the Microbiology lab within 5 days of final culture results.

Notes

Gram stain, identification, and susceptibility tests are billed separately from the culture.

Aerobic culture

Organism(s) recovered in culture may be identified by MALDI-TOF. Identification by MALDI-TOF was developed and its performance characteristics determined by the University of Colorado Hospital Authority. It has not been cleared or approved by the U.S. Food and Drug Administration. The laboratory is regulated under CLIA as qualified to perform high-complexity testing. This test is used for clinical purposes. It should not be regarded as investigational or for research.

Interpretive Data

Positive Gram stains on sterile specimens will be reported to the physician immediately. Positive cultures on sterile tissues or body fluids will be reported to the caregiver on the same day that the results are available.

Final negative results from non-sterile sites are reported at 2 days. 
Final negative results from sterile sites are reported at 5.
