Miscellaneous Outside Lab Test, or MOLT, orders require attention to detail when ordering to ensure patient safety, specimen acceptability, and good clinical care. 

Required fields:

Specimen Type
& Source:

Genetic Testing:If the MOLT is a genetic test, additional information is required.  See this Provider Ordering Job Aid for additional guidance for molecular genetic test ordering.
Test Description:The name of the testing requested.  Be as specific as possible!  Ambiguous or incomplete tests names cause delays in collection, handling, and may prevent samples from being tested. 
Reference Lab:If there is a preferred referral laboratory where testing should be performed, indicate it here. All referral laboratories must be qualified and approved by the Department of Pathology & Lab Medicine.  If you aren't sure whether a laboratory is approved for sending specimens, email before ordering clinlabsendout-oak@ucsf.edu
Test Order ID:This code is provided by the referral lab to indicate the specific test desired.  An Order ID or Test Code must be present before testing can be sent out.  Failure to include a test code will result in delays in TAT and potentially canceled tests. 





Example
In this example, all of the fields are properly filled out and no delays in specimen collection, handling, shipping, or resulting are expected.
