Patient Preparation

Disinfection of the venipuncture site with ChloraPrep. For neonates less than 48 hours of age, isopropyl alcohol is preferred over ChloraPrep.

Collect

BD BACTEC Myco/F Lytic (Myco/F Lytic) bottle. Optimal blood volume, 3-5 mL (range, 1-5 mL)
Draw blood into two Myco/F Lytic bottles, one each from a separate venipuncture.

Unacceptable Conditions

Whole blood in any other container or bottle other than a Myco/F Lytic bottle
Bottles must be received at room temperature (20-25°C). Bottles received at any other temperature are unacceptable.

Storage/Transport Temperature

Room temperature.

Stability (from collection to initiation)

Inoculated blood culture bottles should be transported to the Clinical Microbiology Laboratory at room temperature immediately after collection (if there is a delay in transportation, inoculated blood cultures can be held at room temperature for up to 24 hours before initiation of culture).

Performed

Daily
This test requires prior approval by the Clinical Microbiology Director. It can only be ordered by the Clinical Microbiology Laboratory.

Remarks

Acid-fast bacilli blood cultures are not part of routine blood cultures and require prior approval by the Clinical Microbiology Director. They can only be ordered by the Clinical Microbiology Laboratory.
The Myco/F Lytic bottles are obtained from the Clinical Microbiology Laboratory (ST-730).

Ordering Recommendations

If an acid-fast bacilli blood culture is approved, a requisition form should be completed by provider (incomplete or ineligible requisition forms may delay or prevent the test from being performed).

Requisition form # 44402 (Blue-colored form) is the requisition form used for inpatients; requisition form # 50053 is the requisition form used for outpatients. The requisition form and specimen should be sent to the Clinical Microbiology Laboratory together.

Methodology

Continuous monitoring blood culture. Identification of acid-fast bacilli performed on positive bottles.
Direct examination for acid-fast bacilli not performed on whole blood specimens.

Synonyms

  • Blood Culture for AFB
  • Blood Culture for Mycobacteria
  • TB Blood Culture
  • Culture, Acid Fast Bacilli Blood

Turn Around Time

Positive acid-fast bacillus blood cultures are reported once detected.
Negative cultures are reported at 43 days.
Preliminary reports are issued weekly until completion of the culture (i.e., 1-6 weeks).

Reference Interval

No mycobacteria isolated.

CPT Codes
