Patient Preparation

If patient needs to be scheduled for an appointment for an ACTH Stimulation please contact Infusion Center located at Methodist Estabrook Cancer Center (MECC) 402-354-4379.

Collect

Draw 5 mL SST (Heparinized plasma is acceptable) for cortisol at 0, 30 and 60 minutes. If an ACTH level is also ordered, draw 4 mL lavendar (K2EDTA) at 0 minutes. 

Pediatric Collection

0.5 mL serum or li-heparin plasma for Cortisol
0.5 mL EDTA plasma for ACTH

Storage/Transport Temperature

Cortisol: Minimum 1 mL serum or plasma per collection time. Refrigerate 2-8°C.

ACTH: Minimum 0.5 mL EDTA plasma in an ARUP Standard Transport Tube. Transport FROZEN.

Remarks

Order as individual Cortisol tests indicating time and date of draw. Baseline cortisol specimen need not be fasting but is recommended to be drawn 1 hour or less prior to injection of cortrosyn (0.25 mg). Post-injection specimens are drawn at 30 and 60 minutes.

Cortisol (AM: 6.2-29ug/dL, PM 3.0-17.3 ug/dL, Critical: <2.0 ug/dL)

NOTES: Samples should not be taken from patients receiving therapy with high dose biotin (i.e. > 5mg/dl) until at least 8 hours following the last biotin administration.

Interpretive Data

Interpretive data added to 30 and 60 minute results: Normal peak serum cortisol is greater than 20.0 ug/dL 30 to 60 minutes after 0.25 mg Cosyntopin IV or IM.
